PDF is for printing, without fillable fields. Word tables are editable; neither format calculates totals. Excel: eight lines, CAD only. Quantities: 0–10,000, up to three decimals. Rates: 0–10,000, up to two decimals. Each line rounds half up to cents. No tax, discounts, shipping, deposits or payment balance calculations. Review inputs and totals.

A practical example
Fictional example. Replace the details and rates before use. CAD rates are illustrations, not price recommendations.

| General dental consultation — fictional service | 1 | visit | 80 |
| Routine cleaning — sample fee | 1 | service | 110 |
What to fill in
- Keep clinical records and sensitive patient details outside this tool.
- No insurance coding, claim submission or regional compliance is provided.
Show the fees within an appointment
A consultation, cleaning and any separately agreed service can appear as distinct rows. If the visit has one inclusive fee, describe what it covers instead of adding those same services again as extra charges.
Separate the visit from a wider treatment plan
Bill the services covered by this invoice rather than listing a whole planned course as though it has all been delivered. When several dates are billed together, make the date for each charged service visible so the recipient can reconcile the bill.
Dental invoice questions
Does this include dental claim codes?
No. The template does not provide insurer procedure codes or electronic claim submission.
Can several appointments appear on one invoice?
Yes. Date each service row so the recipient can tell which charge belongs to which appointment. Include only the billing details needed for that invoice.
Is the PDF a treatment record?
No. It is an ordinary invoice and does not replace clinical records, consent or insurer documentation.
