Download an offline template
Blank business details, editable work items and no example prices. One offline layout, separate from the online Simple, Modern and Branded styles.
PDF is for printing, without fillable fields. Word tables are editable; neither format calculates totals. Excel: eight lines, CAD only. Quantities: 0–10,000, up to three decimals. Rates: 0–10,000, up to two decimals. Each line rounds half up to cents. No tax, discounts, shipping, deposits or payment balance calculations. Review inputs and totals.
Choose “Use this template” to edit online and export a completed PDF. Offline downloads leave your browser draft untouched.
A practical example
Fictional example. Replace the details and rates before use. CAD rates are illustrations, not price recommendations.
| Description | Quantity | Unit | Rate (CAD) |
|---|---|---|---|
| General consultation — fictional service | 1 | visit | 90 |
| Administrative service | 1 | item | 15 |
Illustrated subtotal: CAD 105.00. Before tax, discounts or shipping; not a deposit or payment balance.
What to fill in
- Use only the billing details needed for an ordinary invoice; do not enter health information.
- This general invoice does not provide insurance claim fields or regional compliance assurance.
- Project / PO reference
- DEMO-BILL-001
- Service period
- 2026-10-01
Match each fee to its service date
For an invoice covering several visits, date the individual service rows so each charge can be checked against the appointment. The invoice date records when you issue the bill; it should not stand in for the service date.
Distinguish one appointment from several fees
A visit can include more than one separately billed service. Use a row for each agreed fee, but do not add a consultation again if it is already included in a package. Check the intended recipient and billing reference before sharing the invoice.
Medical invoice questions
Can this replace an insurance claim form?
No. This is an ordinary invoice without insurance coding, claim submission or clinical compliance validation.
What service description should I use?
Use the minimum billing description needed. Do not put clinical history or unnecessary sensitive information in a browser-local invoice.
Can I apply different taxes to different procedures?
The editor has one document-level tax calculation. Mixed tax treatment needs a separate review or a different billing workflow.
